Eligibility Information

1. Athlete Physical (mandatory completion before participation)

• Obtain physical form from any of the coaches or online

• The physical must be completed by a physician, signed by the parent/legal guardian and turned in to Coach Degruy or Coach Torres. 

2. AKTIVATE pre-participation forms (mandatory completion before participation)

• Please Click link to obtain instuctions Aktivate Instructions.
• You don't have to print this document. Coaches have direct and immediate access to completed athlete paperwork. 

 3. Coaches will check grades WEEKLY.

• Students should pass every class with a minimum score of 70.
